Causes of Downtime and Data Loss

There are numerous surveys of causes of downtime and data
loss. Here are two. Our own opinion is that human error tends to be
underreported and hence understated. We believe that more effort should be
made to prevention of human errors. These involve training,
process, and backup. Checklists
are useful in understanding and modeling
your downtime.
When it comes to data loss, the large hardware component should
be addressed immediately as part of an upgrade
program, since it is easy and cheap to reduce this cause of
data loss significantly. (The number here is large since most businesses
are careless and too trusting of their non-redundant hardware.)
